They range in size from a few millimeters to several inches in diameter. Hives can be round, or they can form rings or large patches. Hives can occur on any skin surface, but usually spare the palms and soles of the feet. Hives are classified as acute or chronic depending on the length of the episode. Hives are raised, often itchy, red welts on the surface of the skin. Hives may be associated with dramatic swelling reactions; swelling of the lips, eyes, and ears can suddenly and grotesquely alter the appearance of an allergy victim. Swelling of the lips and tongue may occur immediately after eating a food and may be life-threatening because of airway obstruction. Some patients get hives occasionally only when they ingest a specific food or food additives. Other get hives as a chronic problem which can go-on for years. Most studies of chronic hives suggest a low % of food allergy causes usually because diet revision attempts were inadequate to reveal the hidden food causes. Swelling from angioedema can also occur around your hands, feet, and throat.

Hives and angioedema form when, in response to histamine, blood plasma leaks out of small blood vessels in the skin. Hives and angioedema can happen at any age. Angioedema typically causes swelling of the lips, larynx (producing hoarseness or shortness of breath), or the lining of the stomach and intestines (causing abdominal pain). Symptoms include muscle soreness, shortness of breath, vomiting, and diarrhea. Urticarial disease is thought to be caused by the release of histamine and other mediators of inflammation (cytokines) from cells in the skin. Histamine causes fluid to leak from the local blood vessels leading to swelling in the skin. In rare cases urticaria and angioedema are accompanied by shock and difficulty breathing. This is called anaphylaxis. Ordinary hives may be widespread and disturbing to look at, but the vast majority of cases do not lead to life-threatening complications. Sometimes hives are caused by a physical stimulus like contact with heat, water, or pressure. Hives can also be caused by contact with substances that are irritating. Just about any substance has the potential to be irritating.

The treatment of hives depends on the severity of the symptoms. Psychological treatments such as stress management can sometimes lessen severity and occurrence. The most commonly used oral treatments are Non-sedating antihistamines, including Allegra, Claritin, Clarinex, and Zyrtec, are also used to treat hives, expecially hives that last longer than 6 weeks. Chronic hives may be treated with antihistamines or a combination of medications. When antihistamines don’t provide relief, oral corticosteroids may be prescribed. Oral steroids (prednisone, Medrol) can help severe cases of hives in the short-term, but their usefulness is limited by the fact that many cases of hives last too long for steroid use to be continued safely. Other treatments can sometimes include doxepin (Sinequan), an antidepressant that can work as a potent antihistamine, montelukast (Singulair), and medications such as ranitidine (Zantac) or cimetidine (Tagamet), which are more commonly used to treat reflux. To mangage hives and swelling avoid hot water (use luke warm), gentle or mild soap, apply a cool compress or wet cloths to the affected areas, enter a cool room, wear light weight clothes. Avoid irritating the area with tight-fitting clothing. Apply calamine lotion.

Calamine lotion is a proven way to soothe the skin and decrease itching. Antihistamines taken orally or applied topically are among the most widespread treatment options for such skin rashes. These include Paeonia and Forsythia extract, which can be found in a variety of dietary supplements. These can be triggered by extreme temperatures, acute stress, prolonged exposure to sunlight or a sudden increase in body temperature due to exercise or climate. By reducing daily stress and physical exertion, urticaria hives can be minimized and chronic urticaria can dissipate. Keeping the body hydrated is an important component to keeping it cool, and this can help prevent urticaria hives. Identifying and preventing the cause of urticaria is the most important step to keeping the number and intensity of chronic urticaria incidents to a minimum.

Herbal Treatment for Chronic Urticaria

Urticaria, commonly known as "hives" typically develops as rash on the skin, accompanied by severe itching. Acute urticaria is of short duration, up to six weeks, and is usually caused by certain food products, medicines, insect bites and infections. This usually responds well to standard treatment like antihistamines.

Chronic urticaria is usually of much longer duration, and in most cases, the offending cause is not known. Standard modern medicine may not be very helpful here. It is in such patients that Ayurvedic herbal treatment may be very useful and effective. One major advantage of Ayurvedic herbal medicines in such an affliction is that medicines may be taken for long periods without any serious side-effects.

I shall now discuss the different types of urticaria, from the point of view of Ayurveda, and mention their Ayurvedic treatment in brief.

When disturbed "Vata" dosha is mainly responsible for causing urticaria, a pricking sensation is predominant, and this type of urticaria is called "Sheetapitta". When disturbed "Kapha" dosha is mainly responsible, itching is the predominant symptom, and this type of urticaria is called ""Udarda". Impurities are formed in the "Rakta" dhatu due to improperly induced emesis (vomiting) , and the resulting urticaria is termed as "Kotha". When this kotha becomes recurrent, it is termed as "Utkotha".

For the treatment of urticaria, application of Indian mustard (Brassica juncea) oil and fomentation with hot water is beneficial. Induced vomiting and induced mild purgation are beneficial, where symptoms are severe, however, these procedures should be carried out by qualified and experienced physicians.

All efforts should be made to try to avoid the offending cause, if known. Soda bicarbonate powder mixed in warm water, when applied to the hives, gives immediate relief from itching. Ayurvedic medications containing Trikatu or Marich(Piper nigrum) and Turmeric (e.g. Haridrakhand Yog) are useful in controlling urticaria. Other products like Sameerpannag, Sutshekhar, Arogyavardhini, and Gandhak Rasayan are also very useful. Medicines like Kamdudha, Mukta and Gairik are useful in reducing the burning sensation associated with itching in urticaria.

Patients with chronic urticaria may not respond readily to the above mentioned treatment. A detailed history for the symptoms, and a detailed study of the constitutional framework of the patient is essential in deciding the ideal combination of medicines for each patient, so as to get maximum results. Treatment for the impurities caused in the "Rakta" Dhatu, and treatment for stress, are two important principles of treatment which contribute significantly to the success of treatment. Therefore, medicines like Sariva ( Hemidesmus indicus), Manjishtha (Rubia cordifolia), Guduchi (Tinospora cordifolia), Neem (Azadirachta indica), Brahmi (Bacopa monnieri) , Jatamansi (Nardostachys jatamansi) and Shankhpushpi (Consecora decussata) are useful in chronic urticaria. Ultimately, treatment has to be tailor-made for each patient having chronic urticaria.

Raised red and white patches appear on the skin. They are accompanied by burning,intense itching, and stinging. Rubbing and scratching usually aggravate the condition. The outbreak of urticaria is sudden and the disease may affect any part or the entire body. The eruptions may be as small as pin heads or as large as a rupee. The eruption may fade in a few minutes or an hour in one place, but may appear in another.

Urticaria pigmentosa is most often seen in children, but it can occur in adults as well. Rubbing of a lesion produces a rapid wheal (a hive-like bump). Younger children may develop a fluid-filled blister over a lesion if it is scratched.

Lastly, some forms of urticaria and angioedema are related to other diseases, such as other auto-immune diseases (such as lupus and rheumatoid arthritis), certain cancers, chronic infections such as viral hepatitis, and some hereditary forms.

Hives usually subside in one to seven days. If you know the cause and can avoid it, that is best. If the cause is not obvious and you are taking medication, any medication that your physician has told you is not essential should be stopped until the condition has disappeared. We know that allergies to foods and drugs can cause hives, but almost always hives caused by foods or drugs stop by themselves within a few days or weeks.
A large histamine release from these extra mast cells may cause flushing, headache, diarrhea, a rapid heartbeat (tachycardia), and even fainting. This is uncommon with urticaria pigmentosa but may be seen in other forms of mastocytosis.

Types

There are two main forms of hives, differentiated by how long the rash persists.
Acute urticaria is most likely caused by an allergy that is relatively a short-term thing, although it can still take some weeks to resolve in some circumstances.
Chronic urticaria is more long-term, and remissions and relapses often occur.

It is usually safe to take paracetamol and the newer Cox-II inhibitor anti-inflammatories and avoid alcohol. Non-sedating antihistamines (loratidine, fexofenadine, terfenadine, cetirazine, and astemizole) are less likely to cause drowsiness than the less expensive conventional antihistamines. They may be unsuitable in pregnancy. Terfenadine and astemizole may increase the risk of abnormal heart rhythms.They should be avoided if you have heart disease or you are also taking erythromycin, ketoconazole and some other medications.

Antihistamines may relieve some of the histamine-induced symptoms such as itching and flushing. Discuss the choice of antihistamine with your child’s health care provider. With more severe and unusual forms of urticaria pigmentosa, systemic symptoms should be controlled with other medications.

Urticaria, if not responsive to oral antihistamines, may have to be treated with injections of adrenaline or steroids especially for patients ‘complaining of a choking sensation in the throat which occurs due to swelling in the respiratory passage. Suppressive treatment of urticaria is carried out by administering antihistaminic drugs like Chloropheneramine (Avil) triprolidine hydrochloride, Actidil and Cyproheptadine hydrochloride (Periactin). The dose and the frequency should be adjusted to give adequate coverage for 24 hours. Cool the affected area with a fan or an ice pack.
